"High Stakes Clinical Skills Assessment: Validity Challenges and Opportunities," by Dr. Rachel Yudkowsky
In this very timely keynote address, Dr. Rachel Yudkowsky will be speaking on what is needed now more than ever to support validity evidence in high stakes clinical skills assessment. Dr. Yudkowsky is currently the Director of Graduate Studies for UIC.
She served as the Director of the Dr. Allan L. and Mary L. Graham Clinical Performance Center from 2000 - 2018, where she developed standardized patient and simulation-based programs for the instruction and assessment of students, residents, and staff. She also served as Director of the University of Illinois Health Sciences Simulation Consortium from 2009 - 2018.

Session Leaders: Pre-Clerkship and OSCE Note Grading Rubric Interest Group Members
Session Overview: During this showcase, DOCS members will hear recommendations from two working groups exploring best practices in clinical encounter note grading rubrics for use in clinical skills curricula:
The Pre-Clerkship Patient Note Rubric interest group is a collaborative group of DOCS members interested in designing a note grading rubric that can be utilized by the majority of clinical skills courses to assess students’ medical documentation in the pre-clerkship phase of the curriculum. In this presentation, we will give a quick overview of what we have been learning from each other and working on together this year, including identifying appropriate competency levels for the variety of learners who interface with our courses and appropriate anchors for the individual sections of the rubric.
The Clerkship/post-core clerkship Note Rubric interest group came together after a DOCs webinar on note grading rubrics immediately after the announcement that Step 2 CS would not be coming back. In this presentation, we will give a broad overview of what we have been sharing, discussing, and working on for the last several months including: the different components of the post-encounter note that schools have been using, a variety of grading systems that schools have been using for their clerkship, post-clerkship OSCEs, as well as future directions we envision for the post Step 2 CS clinical skills assessment world.

Session Overview: During the synchronous Poster Presentation session, members will choose one of five thematically organized breakout rooms. In these rooms, abstract authors will present their posters, and session moderators will facilitate audience questions. The five breakout room themes include:
- Clinical Skills Assessment and Remediation
- Clinical Reasoning
- Faculty Development and Professional Development
- Clinical Skills Curriculum Development & Innovations: Physical Exam Skills
- Clinical Skills Curriculum Development & Innovations: Communication Skills
The posters will be available on the Members' Area of this website after the session, so if you missed a poster you wanted to view, DOCS members will still have access.

Session Leaders: Committee Chairs and Interest Group Leaders
Session Overview: Have you thought about joining a DOCS Committee or Interest Group? During this session, you will have the opportunity to learn more about DOCS' Committees and Special Interest Groups, as the leaders of these groups will describe the work happening in their group, their goals for the upcoming year, and opportunities for new members.
Logistics: This 50-minute session will consist of two (25-minute) rounds. During each of these rounds, attendees will select a breakout room for the committee or interest group you want to learn more about.
Before the session, feel free to review the descriptions of the DOCS committees and special interest groups to decide which breakout rooms you’d like to join.

Webinar with Dr. Henry Pohl: "A Call To Action: How Can We Collectively Measure The Outcome Of Clinical Skills Education?"
Dr. Henry Pohl leads this webinar discussion about moving forward after the discontinuation of USMLE 2 CS. Dr. Pohl chaired the Standardized Patient Committee of the National Board of Medical Examiners charged with developing a clinical skills examination for licensure from 1989 – 2004. And, he chaired the Test Material Development Committee, USMLE 2CS 2006-2009. During the webinar, Dr. Henry Pohl shares his perspectives on the history of the USMLE 2 CS, implications of its discontinuation, and opportunities moving forward. This session included time to answer presubmitted member questions.

This showcase is a panel discussion, moderated by Katharine Yamulla, who explores attendee questions with the innovation presenters. The showcase is presented in a multi-media format, with DOCS members submitting innovative videos about their curricular innovations ahead of the Showcase.

Workshop: The Core + Clusters Model for Integrating History Taking, the Physical Exam, and Clinical Reasoning: How does it work? And can it work at my school?
Workshop Leaders: Drs. Ron Silvestri, Lynn Kosowicz, Asra Khan, Deepu Gowda, Ani Gangopadhyaya, and Jim Blatt
This workshop describes the impetus behind the creation of the Core + Clusters pedagogical model. Attendees will first be introduced to the rationale for teaching students a streamlined core physical exam, and then they will learn what diagnostic clusters are, how they are taught, and how they foster students’ clinical reasoning. Participants will have ample time in small groups with faculty to either go into greater depth on the core exam, or to delve more deeply into how specific clusters are created to teach history, exam maneuvers, and clinical reasoning on particular symptom presentations. Workshop faculty will share implementation insights from their experience, following which the workshop will conclude with a discussion of potential areas for further development of the Core + Clusters model and research questions that are fertile ground for collaboration.

Webinar: Keys to Successful Clinical Skills Consortia: 4 Models
Now that the USMLE Step 2 Clinical Skills exam has been eliminated, there is an even more pressing need for medical schools to collaboratively establish clinical skills standards and implement joint assessments. This webinar is an opportunity for DOCS members to learn more about the benefits, challenges, and keys to develop and participate in regional consortia that share resources, to ensure competent clinical skills development of their medical students. This webinar includes presentations from four consortia, followed by a moderated discussion, an opportunity for audience questions, and breakouts to meet regional members and initiate dialogue towards implementation of a consortium.

Workshop: Longitudinal Competency-Based Assessment
Workshop Leaders: Candace Pau, MD, Christine Tsai, MD, Athina Vassilakis, MD, and Beth Barron, MD
In this workshop, we will share processes and brainstorm optimal approaches for the development and implementation of competency-based clinical skills assessment. Workshop leaders will discuss their experiences with competency-based assessment through different lenses, including systematic longitudinal assessment, progress testing, and gateway assessment at different timepoints in the curriculum. Participants will then work in small groups to identify potential solutions for challenges and barriers associated with the transition to competency-based assessment, such as selecting appropriate competencies for assessment, establishing expectations for performance at different stages in the developmental trajectory, and optimizing reliability and validity. The workshop will conclude with participants reconvening to review common themes and reflect on opportunities for collaboration and scholarship.

Workshop: Clinical Skills Coaching: A Plan for Diagnosing the Learner, Remediation, and Feeding Forward
Workshop Leaders: Megan Fix, MD; Camilla Curren, MD; Nils Broslin, MD
This workshop is designed to introduce and discuss how remediation strategies for common clinical skills deficits can be implemented at your medical school. First, we will provide an overview of the components of an effective clinical skills coaching program. Second, we will review several vignettes characterizing common learner scenarios which could benefit from clinical coaching. This includes the process of diagnosing the learner deficit and selection of remediation activities to foster clinical skills improvement. Lastly, we will discuss how to make a coaching plan to help feed forward and improve the learner’s performance throughout medical school. We will consolidate takeaway points and allow time for questions and answers.
